Se necesitan su tarjeta de seguro y una identificaci\u00f3n con fotograf\u00eda. Se aceptar\u00e1 una carta de su compa\u00f1\u00eda de seguros que contenga informaci\u00f3n sobre su cobertura m\u00e9dica si no tiene una tarjeta de seguro vigente.<\/p>

Personas con Medicaid, CHIP, o Medicare: <\/strong><\/p>

Se necesitan su tarjeta de beneficios y una identificaci\u00f3n con fotograf\u00eda. Los padres de ni\u00f1os con Medicaid\/CHIP deben presentar la tarjeta de Medicaid\/CHIP, el certificado de nacimiento del ni\u00f1o, y la identificaci\u00f3n con fotograf\u00eda de los padres.<\/p>

Fixed discounts are based on income and number of dependents in the household. Persons without or with private insurance, Medicaid, CHIP, and Medicare who wish to take advantage of this discount are required to provide proof of identification and income described below.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

\u2022\tTarjeta de identificaci\u00f3n de TX \u2022 Tarjeta de identificaci\u00f3n del consulado
\u2022\tTarjeta de Registro de Elector \u2022 Documentos de Residente Legal<\/p>\n

Si no hay identificaci\u00f3n con foto, se requieren dos (2) de los siguientes:<\/strong>
\u2022\tActa de Nacimiento \u2022 Expediente Escolar
\u2022\tTarjeta de Seguro Social \u2022 Licencia de matrimonio
\u2022\tTarjeta de Registro de Elector \u2022 Tarjeta de Servicio Selectivo, etc.<\/p>\n
