MANO A MANO COMMUNITY PROMOTORA PROGRAM

 

Promotoras de Salud are trusted members of the community that share many of the same social, cultural and economic characteristics.  They are a vital component in linking patients to community resources and medical services to achieve positive changes and good health. As part of the medical team they help patients better manage their chronic conditions. 

 

Promotoras work with multiple community partners to identify and facilitate services for Migrant Seasonal Agricultural Worker Families

The Promotoras (Community Health Workers) serve as:

 

  • Advocates
  • Mentors
  • Educators
  • Translators

Outreach & Enrollment Representatives

Consist of Certified Application Counselors ready to assist the community with the application process to the Health Insurance Marketplace and Your Texas Benefits.  They participate in community outreach providing information for application assistance to benefits such as affordable Health Insurance, Medicaid, CHIP, TANF, SNAP, and Renewals.

Our dedicated community outreach team hosts events promoting the health and wellbeing of the community such as:

  • Holiday Activities for Children with Special Needs
  • Migrant Seasonal Agricultural Worker Health Fairs

 

We also participate with partnering agencies to provide outreach and education.

 

Patient Advocate Representative Collaborates with the entire community outreach team:


The Patient Advocate works as a liaison to facilitate the patients visit. They assist by helping the patient navigate the Center’s overall services and processes.

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
